This form can be accessed on the HSE website in the document entitled The Medical Examination and Assessment of Divers (MA1). The form is on page 26 which you can access <a href=\"http:\/\/www.hse.gov.uk\/diving\/fact-sheet-and-medical-questionnaire.pdf\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\">here<\/a>. Your GP cannot proceed with the actual medical examination unless he is an AMED.
