On your first HSE diving medical you will require a completed medical questionnaire signed by your GP. This form can be accessed on the HSE website in the document entitled The Medical Examination and Assessment of Divers (MA1). The form is on page 26 which you can access here. Your GP cannot proceed with the actual medical examination unless he is an AMED. See the DIVER FACT SHEET from the same HSE document.
